To receive your $1.50 per case promotional allowance on all purchases, please download, print and complete the rebate form and submit with copies of your distributor invoices, indicating the number of cases purchased in the quarter you are redeeming and the total amount of your rebate. Product must have been purchased during the previous calendar quarter. Payments must be postmarked no later than 30 days from end of previous calendar quarter.
